Setting: Domiciliary care providing regulated activity including personal care and treatment of disease, disorder or Injury. We cover all age groups in home based care.
Overview of current role
In my role I have responsibilty for all the care clients we recieve and staff working within peoples homes. This includes oversight of care from health and care professionals coming into a persons home.
I oversee our organisational and service compliance with registration and regulation. As an organisation we have a comitment to individual care pathways and this often means negotiating across the system in health, care and other agencies working towards a shared care vision. Lots of collaboration involved in this role and translating comissioning and packages of care including where appropriate delegation of healthcare activities.
I also have a role in the education and training part of our workforce including supporting apprenticeships and career development.
Specific health and care activities include:
- personal care
- palliative care and support
- nutrition and hydration
- medication support
- respite service
- nursing oversight
- support discharge
- complex care
- support to attend school or daycare
- bespoke packages of care.
How did I get here
I started my career as a ward staff nurse and then community staff nurse, district nurse, community matron, care home matron and discharged support community matron moved to social care in this current role.
Could see lots of gaps when people come out of acute setting and navigating the social care setting is complex. I wanted to work in a role that supported that transition. Worked with many social care nurses in my career and wanted to have the opportunity to influence outcomesand experience of people.
